Our Neuro 4 hire form and the terms and conditions associated with the hire of the unit are available via the link in a Word Document format . A few points to note are:-
- Neuro4 machines are available to hire complete with one treatment card and the relevant accessories.
- Facial accessories are 1 pair of leads, gel and tape.
- Non facial accessories are 4 leads and 2 packs of 4 pads.
- Neuro4 units can only be hired by the patient; they cannot be hired by a relative (unless it’s by a parent/guardian on behalf of a minor). Hire agreements cannot be entered into with any organisation including, hospital, PCT, private physiotherapy clinics or solicitors although there is an option for units to be purchased by these companies.
- Neuro4 units can only be hired to UK residents, but are available for purchase by non UK residents.
- The use of a Neuro4 unit by a patient must be properly supervised and monitored. Whilst the treatment is to be completed daily at home you will need a therapist to help with chip and programme selection, to advice you on electrode placement and to monitor your progress and change the treatment as required. There is a section on the hire form for the details of the therapist who will be providing you with treatment guidelines and monitoring your progress. On occasion we may need to verify these details.
- All forms returned to the clinic must have both signature sections completed – to begin the hire contract and to confirm that you are treating for a medical reason and therefore no VAT needs to be charged.
- You can send a cheque with the form or ring the clinic to pay by credit card, the unit will be dispatched in 1 – 2 working days following receipt of both payment and hire form.
- If you or your physiotherapist requires some assistance with microcard selection then please contact the clinic.
- If you do not have a physiotherapist but are interested in pursuing treatment then please contact the clinic and we will discuss appointment options with you.
- We will send hire reminder letters at the beginning of each month, but you are responsible for arranging prompt payment or safely returning the unit to the Lindens Clinic.
